All endodontic cases must be assessed for level of complexity. Three levels of difficulty exist: Minimum, Moderate, and High.
Pull up the “General Referral” form (REFERL), fill out the Endo tab completely and get faculty approval.
THIS FORM MUST BE SWIPED PRIOR TO ALL ROOT CANAL TREATMENTS REGARDLESS OF WHERE THEY ARE PERFORMED.
After a case has been deemed appropriate for the Endo resident clinic by an Endo faculty, or for you to
see the patient in the so-called embedded area in the second floor specialty clinic, please make sure that
the form shown above is filled out.
Then inform the patient that the Endo coordinator for the endo residency will be contacting them for
an appointment. To initiate this, you have to send an email to Zandro Alcantara (zalcanta@PACIFIC.EDU)
and cc’ Erika Rincon (erincon@PACIFIC.EDU) listing the patients’ name, axiUm number, the involved
tooth and any pertaining details and also whether financials were discussed.
Please make sure that you follow up with patient and Zandro if you do not see the patient scheduled in
a reasonable amount of time.
